Tribe Permission Slip

I grant Oasis Church’s leadership permission to take my child on a community outreach event (off church grounds) on Dec 21, 2022. I understand this will/may require transportation via personal vehicles and walking some local neighborhoods, under adult supervision.

By submitting this permission slip, you are agreeing to allow your child to participate and agree to not hold Oasis or Tribe or any of its leadership legally liable.

Parents, please note: We will feed the students as normal at 6 pm but will plan to leave church grounds at 6:15pm. We will return no later than 8:15pm. Please have your child/children dress appropriately for extremely cold weather. ***All students must have a submitted permission slip by their parents to be allowed to participate tonight as no one will be left at the church during the outing.
